Right at Home Collin County (4) - JOB SUMMARY
The Registered Nurse provides quality nursing care in compliance
with physician orders, an established care
plan, Right at Home policies and procedures, and all other laws and
relevant professional standards.

KEY ACCOUNTABILITIES
Judgment/Decision-Making
- Completes an initial assessment of client and family to
determine home care needs, making necessary revisions per physician
orders as status and needs change
- Performs assigned nursing procedures, including administration
of treatments and medications in the care of the client in
accordance with the client care plan, nurse delegation, the State's
Nurse Practice Act and physician orders
- Demonstrates the ability to think critically in exercising
professional and clinical judgment

Verbal Communication

- Communicates with client's physician regarding the client's
needs, changes in condition, or when obtaining physician's orders
as needed
- Provides education to clients and family members on resources,
medications, self-care techniques, and/or medical conditions per
physician orders

- Understands and attempts to meet the needs, wants, and
expectations of clients
- Listens and responds to calls for help from other direct care
staff when needed
- Hears, considers, and digests verbal and nonverbal feedback
from nursing supervisor, clients, and their family members
Analysis Skills
- Observes and reports any signs or symptoms indicative of
changes in condition of the client to the physician and documents
such reports and observations in the client record Updates the care
plan per physician orders.
- Plays a key role in clients avoiding unnecessary visits to the
ER and or hospitalizations
- Identifies discharge-planning needs as a part of the client
care plan implementing them in accordance with physician orders,
and sending the discharge summary to the physician
Assertiveness
- Addresses concerns or complaints voiced by clients, families,
or caregivers and notifies physician and/or nurse manager
immediately
- Demonstrates the ability to interpret relevant information
needed to identify each client's nursing care requirements relative
to his/her needs
- Asks questions to understand views that differ from one's own,
looking for areas of compromise
Coaching/Training
- Acts as a role model for new caregivers, accompanying them on
home visits and/or acting as a mentor as assigned
- Demonstrates flexibility in adjusting assignments to meet the
needs of the care team
Accountability
- Maintains productivity according to Right at Home
standards
- Fosters the professional development of self and other care
staff
- Evaluates own performance with home care standards and
performance review of a nurse supervisor
- Acts as case manager when assigned by the nursing supervisor,
assuming the responsibility to coordinate client care for assigned
caseloads
Written Communication
- Prepares, records, and maintains clinical and progress notes in
the client care record in an accurate, timely manner and per Right
at Home policy and procedure
- Meets required deadlines for documentation of certification,
re-certification, supervision reports, care plan updates,
addressing progress towards goals, and physician orders as
needed
- Develops end of shift communication to oncoming nurse for
continuity of care and ongoing assessment needs

NON-ESSENTIAL FUNCTIONS
- Other general office and clerical functions as
assigned
- Participates in community relations activities as assigned

EDUCATION / SKILLS / ABILITIES / AVAILABILITY
- Registered nurse with current licensure to practice
professional nursing in the state
- 2 years of nursing experience (home health/hospice
preferred)
- Must possess and maintain current CPR certification
- Knowledge, ability, and the necessary skill and judgment to
perform all nursing procedures under the direction of a physician
as outlined in the scope of practice in the State's Nurse Practice
Act
- Meets the requirements for employees providing direct client
care
- Must have a valid driver's license and use of an insured
automobile for work or access to adequate transportation
- Available for work on weekends and holidays as
required
- Read, write, speak and understand English as needed on the
job
- Participate in in-service education programs as mandated by
State law (this requirement need not be met exclusively through
Right at Home sponsored in-services programs)
