Our state-of-the-art technology connects people and processes to achieve the quadruple aim of reducing healthcare costs, improving patient and provider experiences, and delivering better outcomes.The Virtual Care Program Manager will lead a team of clinicians and non-clinicians to deliver cutting-edge virtual care servicesDescriptionResponsibilitiesYou will be managing a team to deliver virtual care services and care management programs leveraging best-in-class virtual care platform and remote patient monitoring (RPM) technology/devices to improve health outcomes for patientsKey Responsibilities:Candidates should be prepared to illustrate competency to work with the clinical team in the practice and partner with technology teams to launch and scale successful virtual care services and care management programs including remote patient monitoring services.Lead and Manage a team responsible for -

• Provider Relationship Management
• Patient intake into the program - Reviewing patients and identifying patients that fit the inclusion criterion for Remote Monitoring
• Answering questions about our Chronic Disease Practice from prospective patients
• Actively enrolling patients into the program at the clinic location
• Discussing the RPM program with patients and onboarding them into the program
• Training patients on the use of at-home vital sign monitoring equipment and mobile application
• Inventory Management: Keeps supplies on hand by inventorying stock and liaising with RPM partners to ensure adequate supply
• Assigning RPM kits to patients onboarding into the program
• Off-boarding patients when necessary, from the program and preparing their devices for the next patient
• Connecting with patients in the Chronic Care Management (CCM) cohort via phone and/or video telemedicine once a month or when they have questions
• Generating reports and notes about the patient's condition and escalating to the appropriate clinical resource within the clinic
• Work with upper management to establish processes for continuous improvement and scale operations
• Set up and monitor the Triage of patient problems and notifications identified by the RPM technology
• Compiling feedback from patients and clinicians about the program and technology
• Ensure all communications within the program maintains HIPAA Compliance
• Collaborate with the technology team to identify gaps and provide recommendations for future roadmaps
• Constantly explaining and selling the benefits of the program to internal and external stakeholdersQualifications
  • This role is best suited to someone who has experience dealing with patients and is dissatisfied with the current state of care, specifically for elderly chronic patients
  • You are self-motivated and a problem solver
  • Understanding of Cloud Technology like AWS, Salesforce, google cloud, etc
  • English Language Fluency - written and verbal required
  • College degree preferred
  • Second language skills will be highly regarded although not required
  • Self-actualized with the ability to work independently
  • Leadership skills to manage and grow a team
  • Deep interest in medical innovation, technology, and making systems better
  • Valid Drivers License (driving record thresholds apply)
  • Reliable personal vehicle - mileage reimbursement provided on rare occasions where you need to visit a patient or long-term care facility
  • Good understanding of the US healthcare system
